Adult Social Worker

We are looking for an Adult Social Worker to join the Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team:

The Hospital Discharge Team supports adults to move from hospital into appropriate care and support arrangements. Working alongside health professionals, individuals and their families, the team assesses social care needs and coordinates support to promote independence and a safe, timely discharge. As an Adult Social Worker, you will contribute to discharge planning, helping to ensure that each person's needs, wishes and wellbeing are central to decisions about their ongoing care.

About you:

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2-year experience is essential to be considered for this role. Must have experience working within a Hospital Discharge Service.
