Community Health Care Lead Practitioner| £31.08 per hour| Sefton

Join a Community Health Care Team based in Merseyside. The Social worker will conduct comprehensive assessments of adults with complex health and social care needs, developing care plans and coordinating appropriate support to promote independence and wellbeing. This role is paying £31.08/hour, 2 days working from home and an initial 3-month contract.

This is a locum opportunity; the social worker is required to have 1 year post ASYE qualification experience. It is beneficial to have previous social work experience within a community setting
