Adult Social Worker

We are currently looking for an Adult's Social Worker to join a Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This team supports adults who have been discharged from hospital, with the aim of helping them return safely to the community with an appropriate care plan tailored to their individual needs.The team works with adults aged 18 and over who may have learning disabilities, mental health needs, and/or physical disabilities. They undertake a range of assessments, including Discharge to Assess, Continuing Healthcare (CHC), and Care Act 2014 assessments. The team is made up of Occupational Therapists and Social Workers who work collaboratively to ensure individuals receive the most appropriate support and care to promote their independence and wellbeing.

About you

The successful candidate will be a enthusiastic and acknowledge within adult social work teams such as, community, Hospital, older people and phsycial disabiltities. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience. A valid UK driving licence and vehicle is preferred for this role but not essential.
