Adult social worker

We are looking for a Mental Health Social Worker to join an Adult Mental Health Team.

This role requires a Social Work Qualification with a minimum of 2 years permanent experience within a Local authority.

About the team

The team works with older adults experiencing complex mental health needs, supporting individuals within the community and during hospital discharge processes. The service works closely with health professionals and partner agencies to deliver strengths-based support and positive outcomes for service users.

The role involves completing Care Act assessments, care and support plans, Mental Capacity Assessments, safeguarding enquiries and Section 117 aftercare planning. Participation in multidisciplinary meetings, ward rounds and duty responsibilities within a fast-paced Adult Mental Health setting is key in this role too.

About you

Experience working within a Community Mental Health Team is essential for this role. Experience working with older adults with complex mental health needs and confidence in undertaking multidisciplinary working with health colleagues and partner agencies.

It would be beneficial to have strong knowledge of the Care Act, Mental Capacity Act and safeguarding legislation.

A degree within Social Work (Degree/DipSW/CQSW)
