Social Worker

We are looking for a Social Worker to join a Mental Health Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.

About the team

This team work to support adults with mental health concerns leaving hospital to ensure they have a safe and comfortable transition back into home living. Working in tune with other connected services such as hospital staff and CPNs is imperative to a service user's safe recovery. Working with a strength-based, MDT approach to this role is key to ensuring discharges are successful.

About you

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience is essential in order to be considered for this role. Experience in a mental health setting (whether that be community based or inpatient) or generic hospital based work is all experience that lends well to this role.
