Social Worker

We are looking for a Social Worker to join a Hospital Discharge Team.

This role requires a social work qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team who work to ensure patients discharges from hospital are safe and that they have as much support leaving the hospital as they need. Use of D2A procedures, completing mental capacity act assessment and dealing with safeguarding procedures are all day to day responsibilities in this role. Advocating for service users and their independence whilst using the conversations model to connect people to their social network is key to the success of each service users discharge.

About you

Experience in a hospital or community based team is experience that lends well to this team. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work setting are essential for this position. It is a requirement that the successful candidate has a valid UK driving licence and access to a vehicle.
