Job description

Wokingham Borough Council is looking for a Senior Practitioner to join their 14-25 Transitions Team. This is a Locum Social Work role.

The ideal Social Worker will have recent experience working in a Learning Disabilities or Transitions team at this level. The key to success in this role is to treat every case individually to create the best possible outcomes for the service users involved.

Your responsibilities as a 14-25 Transitions Senior Practitioner:

  • Carry a complex caseload of vulnerable service users aged 14-25 who have a learning disability
  • Working with service users who are at risk of admission or are in a Mental health placement
  • Use appropriate risk management and MDT approach when working
  • Supporting the vulnerable service users back into the community
  • Advocate for service user independence where appropriate
  • Provide advice and support to others in the team when needed

Requirements of you as a 14-25 Transitions Senior Practitioner:

  • A recognised Social Work qualification (Degree/DipSW/CQSW)
  • A valid Social Work England registration
  • Have competent knowledge and experience of Transitions work
  • A valid UK driving licence and car is preferred for this role

Location: Wokingham, Berkshire

Umbrella pay rate: £37.00 per hour
