Senior Practitioner - CHC Team

The Royal Borough of Greenwich is looking for a Senior Practitioner to join their Continuing Healthcare Team. This is a Locum Social Work role.

The ideal Social Worker will have recent experience with Continuing Healthcare, DHC/ DST work. The key to success in this role is to be able to work confidently from the beginning of the contract whilst being able to aid in the running of the team.

Your responsibilities as a Continuing Healthcare Senior Practitioner:

• Complete CHC assessments and reviews
• Work within the CHC framework
• Complete Care Act Assessments and reviews
• Attend dispute resolution meetings and/or CCG panel
• Comprehensive risk management
• Completion of Mental Capacity Assessments and best interest decisions
• Completion of safeguarding enquires where applicable
• Act as the Duty Senior one day a week to supervise unqualified staff

Requirements of you as a Continuing Healthcare Senior Practitioner:

• A recognised Social Work qualification (Degree/DipSW/CQSW)
• A valid Social Work England registration
• Have competent knowledge and experience of Multidisciplinary/ CHC work

Location: Greenwich, London

Umbrella pay rate: £36.00 per hour
