Hospital Social Worker

We are looking for a Senior Social Worker to join a Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.

About you

The successful candidate will have worked in a hospital team or short term assessment setting. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience working in an Adult's Social Work environment is essential. a valid UK driving licence and car is preferred but not essential for this role.

About the team

This is a fast paced team who work to ensure patients are safely discharged from hospital and have enough support leaving the hospital as they need. Advocating for service users and their independence when safe is always be at the forefront of the teams motivation. Use of D2A procedures is a day to day necessity to make sure full care can be given.
