Hospital Discharge Social Worker

A Hospital is looking for a Discharge Social Worker who will be working with outbound patients.

You will need to be a Qualified Social Worker with post qualification experience and a knowledge on working within Hospital environments ideally.

Your responsibilities as the Social Worker:

• Completing D2A Assessments
• Working towards KPIs
• Undertaking Face to Face clients visits
• Caseholding
• Multiprofessional working
• Completing Safeguarding Assessments
• Evidence based Practice

Requirements of you as the Social Worker:

• A recognised Social Work qualification (Degree/DipSW/CQSW)
• A valid Social Work England registration
• Experience of Safeguarding
• Must hold a full UK Driving Licence

Location: Doncaster (hybrid working)

Salary Scale: £29,447 - £39,513
