Continuing Healthcare Social Worker

A Local Authority in North London is looking for a Locum Social Worker to join their Continuing Healthcare Team. This role is offering £32.00 per hour.

The ideal Social Worker will have recent experience working in a Multidisciplinary, CHC environment. The key to success in this role is to be able to work confidently from the beginning of the contract with little supervision.

Your responsibilities as a Continuing Healthcare Social Worker:

• Work within the CHC framework to complete CHC assessments and reviews
• Complete Care Act Assessments and reviews
• Attend dispute resolution meetings and/or CCG panel
• Completion of Mental Capacity Assessments and best interest decisions
• Completion of safeguarding enquires where applicable
• Present cases to panel when necessary

Requirements of you as a Continuing Healthcare Social Worker:

• A recognised Social Work qualification (Degree/DipSW/CQSW)
• A valid Social Work England registration
• Have competent knowledge and experience of Multidisciplinary/ CHC work

Location: Barnet, London

Umbrella pay rate: £32.00 per hour
