Advanced Practitioner - Hospital Team

A North London Local Authority is looking for an Advanced Practitioner to join their Hospital Team. This is a Locum Social Work role that requires you to be in the office full time.

The ideal Social Worker will have extensive experience of working in a Hospital environment at this level. The key to success in this role is to be able to work confidently from the beginning of the contract with little supervision.

Your responsibilities as a Hospital Advanced Practitioner:

• Ensure that service users are discharged safely
• Undertake Care Act Assessments
• Work collaboratively with service users, their carers and personal support networks to create effective care plans
• Work to set KPI's
• Provide advice and guidance to others in the team when necessary

Requirements of you as a Hospital Advanced Practitioner:

• A recognised Social Work qualification (Degree/DipSW/CQSW)
• A valid Social Work England registration
• Have competent knowledge and experience of Hospital work
• To be based in the office 5 days a week

Location: North London

Umbrella pay rate: £39.00 per hour
