Adult's Social Worker

We are looking for a Social Worker to join an Adult's Transitions Team.

This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.

About the team

The team specialises in supporting young adults with disabilities move into adult services. A key aspect of this role is to aid the service users gain as much independence as appropriate as they move into adulthood whilst ensuring their safety through care and risk assessments. Taking time to create trust and strong relationships with service users is key to the success of the work this team does.

About you

The successful candidate will be well versed in care assessments and learning disabilities work. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience working in a transitions or LD setting are essential for this position.
