Adult's Social Worker

We are looking for a Social Worker to join a Hospital Discharge/ Transfer of Care Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team that work to ensure patients are safely discharged from hospital and have as much support leaving the hospital as they need. Advocating for service users and their independence when safe will always be at the forefront of the teams motivation. The team prioritises working with service users and their carers promote and maintain their dignity, choice and independence.

About you

The successful candidate will have worked in a hospital team or short term assessment setting. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work environment is essential. A valid UK driving licence and car is preferred but not essential for this role.
