Adult's Social Worker

We are looking for a Social Worker to join an Assessment and Intake Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This team specialises in community based work by providing an initial contact service for older people and people with physical disabilities. This is a team that works on short term cases of maximum 6 weeks to start the process of long term care solutions, crisis intervention work, organise respite services and complete placement assessments. Working with connected healthcare professionals such as Nurse Assessors, Occupational Therapists and wider connected services is key to the success this team achieves.

About you

The successful candidate will have a proactive way of working along with extensive experience in a community based Social Work team. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience is essential for this role.
