Adult's Social Worker

We are looking for an Adult's Social Worker to join a Continuing Healthcare Team.

About the team

As a Social Worker in this team, completing face to face visits to assess within the CHC framework is a key aspect of your day to day responsibilities. The team also works on a multidisciplinary plan. You will be required to attend dispute resolution meeting and/or CCG panel and present cases to panel when needed.

About you

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience within adult's Social Work is essential in order to be considered for this role. Experience within a hospital, reviews or community based environment is all experience that lends well to this position.
