Adult's Social Worker

We are looking for a Senior Social Worker to join a Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.

About the team

This is a very fast paced team who work to ensure patients discharges from hospital are safe and have enough support leaving the hospital as they need. Use of D2A procedures is a day to day necessity to make sure full care can be given. Advocating for service users and their independence when safe is always be at the forefront of the teams motivation.

About you

The successful candidate will have worked in a hospital team or short term assessment setting. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience working in an Adult's Social Work environment is essential.
