Adult Social Worker

We are looking for an Adult's Social Worker to join the Hospital Team.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

The hospital discharge team is a fast paced team working to support short term cases. The team is based on site at the hospital, the team works to discharge patients from hospital with the aim to create a care plan for them when they are due to go into community setting.

About you

A Social Work (Degree/DipSW/CQSW) within a minimum of 1 year post ASYE experience is required. The successful candidate will have previous experience of working with relevant legislation. Experience of working with learning disabilities, older people, autism and physical disabilities.
