Social Worker - Hospital Team

My clients are looking for a Social Worker to join their Hospital Discharge Team. This a locum social work position based in Central London. The social worker will be working on site five days a week. The Social Worker will be working to KPI's, Carrying out Care Act assessments 2014, and MCA's.

Your responsibilities as an Social Worker

• Implement care plans and care packages
• Carry out Care Act 2014 assessments
• Carry out MCA assessments
• Undertake Section 42 enquires
• Carry out the best level of care and services for the benefit of the service user
• Liaising with other practitioners
• Carry out Dol's Reviews
• Work to a busy environment
• Work towards KPI's

Requirements of a Social Worker

• Current registration with Social Work England
• 2 years post qualifying UK experience
• Qualified Social Worker BA/DipSW/CQSW or equivalent
