Social Worker - Hospital Discharge Team

My client is looking for a Social Worker to join their Hospital Discharge Team. This post is based in West Sussex, the social worker will be working to a flexible working from home and in the office style. The client is offering £150 expenses allowance to Social Worker who live outside of the borough.

Your responsibilities as an Social Worker

• Implement care plans and care packages
• Carry out Care Act 2014 Assessments
• Carry out Mental Capacity Assessments
• Undertake Section 42 enquires
• Attend meetings as a team for the benefit of the client
• Working towards KPI's
• Carrying out D2A assessments
• Carry out CHC checklists

Requirements of a Social Worker

• Current registration with Social Work England
• 2 years post qualifying UK experience
• Qualified Social Worker BA/DipSW/CQSW or equivalent
• Must be double Vaccinated for this post
