Social Worker - Hospital

A Local Authority in Buckinghamshire is looking for an Social Worker to join in their Hospital Team. This is a Locum Social Work role.

The ideal Social Worker will have knowledge and experience working in a hospital environment. The key to success in this role is to be able to start a contract confidently without the need for supervision.

Your responsibilities as a Hospital Social Worker:

• Ensure that service users are discharged safely whilst working to set KPI's
• Facilitating discharges from hospital when short term services are in place to support their move home
• Assessing service users leaving hospital that have long term service plans in place (D2A)
• Undertake Care Act Assessments and Mental Capacity Act Assessments
• Liaise with other services within the council for the best intertest of the service user

Requirements of you as a Hospital Social Worker:

• A recognised Social Work qualification (Degree/DipSW/CQSW)

• A valid Social Work England registration

Location: South-East England, Buckinghamshire

Umbrella pay rate: £38.00 per hour
