Social Worker

Charles Hunter Associates are looking for an Adult Social Worker for the Hospital Discharge Team.

To apply for this role, you must have a Social Work Bachelor's Degree is Registered with Social Work England, have completed an ASYE Year and 2 years Post Qualifying experience.

About the team:

As an Adult Social Worker within the Hospital Discharge Team, you will be working with adults within a hospital setting, conducting Care Act 2014 Assessments and focusing on the service user's needs.

You will be working alongside a partnership with other professionals in the service, as - well as a stable and supportive management team.

About you:

The successful candidate will be a passionate and dedicated professional, with a strong working knowledge of Adult Social Work Legislation. It would be beneficial for you to have at least 3 years' frontline Adult's experience.
