We are looking for a passionate, hardworking Social Worker to join a Council in the East Midlands as a Mental Health Inpatient Link worker Role (working age adults)

Role as a Care Manager/ Social Worker:

• Competitive pay rate of £38.20 per hour
• promoting and safeguarding the rights of patients, ensuring capacity considerations are embedded throughout the admission.
• Fulfilling the Councils responsibilities under the Care Act 2014, including Prevention and conducting Safeguarding Enquiries
• Providing a social perspective to inpatient assessment, enabling positive risk taking in relation to discharge with a discharge to assess approach.
• Supporting the virtual discharge hub in understanding the roles and function of social care, and supporting efficient referral pathways, support planning and funding requests
• Provide feedback to senior colleagues on matters in relation to flow and barriers to discharge.

To be considered for this Social Work position, you must have

• Valid Social Work England Registration

• A recognised Social Work qualification (Degree/DipSW/CQSW) with AYSE year completed
