Adult's Social Worker

We are looking for a Senior Adult's Social Worker to join a Hospital Team.

This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.

About you

The successful candidate will be well versed in hospital discharge procedures and have good IT skills of LAS/IAS and google an advantage. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years' experience working in a hospital setting are essential for this position.

About the team

This team specialises in discharges using D2A from Kingston Hospital and all Out of Borough Hospitals. The team works closely with external organisations - statutory and voluntary on a day-to-day basis.
