Adults Social Worker

We are looking for an Adult's Social Worker to join the Hospital Discharge Team.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

The hospital team is a fast paced environment, this team works with 18+. The team will deal with a range of clients where needs will vary from physical disabilities, older people, learning disabilities and mental health. This team will aid to support those being discharged from hospital and look to put together care plans to further assist their needs.

About you

The successful candidate will have previous experience of working with relevant legislation. A Social Work (Degree/DipSW/CQSW) within a minimum of 1 year post ASYE experience is required. Experience of working with learning disabilities, older people, autism and physical disabilities.
