Adult Social Worker

We are looking for an Adult Social Worker in the Home First and Urgent Response Team

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast-paced team in a busy working environment. The team consists of highly experienced Social Work Professionals in hospital discharge and admission avoidance. The aim of the team is to ensure safe discharges from hospital for each individuals as well as offering support resources to family and carers. The team work in office 2 full days weekly, with urgent visits being carried out as necessary.

About you

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience is essential in order to be considered for this role. It is essential to hold a full valid UK driver's license with access to a car. Experience or ability to adapt to multi-disciplinary working in which evidence to prove this would be preferred.
