Adult Social Worker

We are looking for an Adult Social Worker to join the Hospital Discharge Team.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

The hospital discharge team is a fast paced working environment, this team works with those who need to be discharged from the hospital. This team is a very supportive team that ensure regular supervision is being taken place for the purpose of the clients. Responsibilities within this team will include, carrying out discharge to assess assessments, liaising with families of the patients and putting together a care plans.

About you

A Social Work (Degree/DipSW/CQSW) within a minimum of 1 year post ASYE experience is required. Experience of working in hospital, community, first response or complex care is transferable for this team.
