Adult Social Worker

We are looking for an Adult's Social Worker to join the short term reablement service.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

The reablement service is a fast paced environment where the aim is to support the transaction of cases to long term packages of care through to locality teams. This is a highly proactive team that prides itself on their team work with other agencies across the borough. They work to support occupational therapists and physiotherapists with care act assessments and mental capacity act related tasks.

About you

You will be a passionate Social Worker with extensive experience and knowledge of Adult's Services. You will have practice of carrying out Mental Capacity Act assessments, Care Act assessments and working under section 42. You must have a qualified social work degree with a minimum of 1 year post ASYE. This team does work with Liquid Logic, experience of this system will be highly desirable.
