Adult Hospital Social Worker

We are looking for an Adult Social Worker to join the Hospital Discharge Team.

Requirements

Must have a Social Work Degree with 1 year post ASYE experience

About the team

The hospital discharge team is a fast paced environment, this team works with those who have been admitted in hospital. This team is designed to aid those to be discharged from the hospital back into the community for further ongoing support. This is a highly effective team within the service that works hard to ensure a smooth transitions for those individuals.

About you

You will have a Social Work (Degree/DipSW/CQSW) within a minimum of 1 year post ASYE experience is required. You will be carrying out Menta Capacity assessments, Care Act 2014, and section 42 assessments. Prior/ or current experience of hospital, complex care, front door and community will be desired or this post.
