Cambridgeshire County Council are looking for an experienced Social Worker to join their Discharge to Assess Reviews Team. This is a Locum Social Work role.

The ideal Social Worker will have recent experience working in a Hospital or Reviews position. The key to success in this role is to work confidently from the beginning of the contract with little supervision.

Your responsibilities as a Discharge to Assess Reviews Social Worker:

  • Work closely with the community to ensure that service users are discharged safely
  • Deal with safeguarding enquiries
  • Complete reviews on care plans and packages
  • Undertake Social Care Assessment within the community following discharge from hospital.
  • Liaise with Service users support networks and other services within the council for the best intertest of the service user
  • Work to set KPI's
  • Complete some duty work

Requirements of you as a Discharge to Assess Reviews Social Worker:

  • A recognised Social Work qualification (Degree/DipSW/CQSW)
  • A valid Social Work England registration
  • Have competent knowledge and experience of Hospital and Reviews work

Location: Cambridge, Cambridgeshire

Pay rate: £34.00 per hour
