Locum Senior Practitioner - Leaving Care - Havering

Havering Council is currently looking for a Locum Senior Practitioner to join their Leaving Care Team based in Romford.

You will work with service users aged between 16-21 year's old and be responsible for assessments, placements, independent living to working with other professionals to put care packages together.

You should have a strong interest in young people and a commitment to change lives.

Experience of dealing with mental health issues desirable.

You must have:

  • Minimum 3 year's post qualifying experience
  • Proven Leaving Care experience
  • HCPC registered
  • Enhanced DBS, with list checks

Role and responsibilities:

  • To develop pathway plans for 16 plus
  • To complete assessment of need for young people aged 16
  • To manage complex young people through transition to adulthood
  • To support young people to develop appropriate independence skills
  • To promote positive outcomes for young people in all areas of development
  • To promote education and training
  • To engage young people in service delivery and training
  • To engage with complex issues which will include mental health, substance misuse and criminal activity
