Liverpool City Council is currently looking for an experienced and ambitious Community Care Co-ordinator to join their exciting adult mental health team.

The ideal candidate will cover an existing caseload of service users with severe and enduring mental health conditions and will act under the CPA Framework.
The Community Care Co-ordinator will also be undertaking statutory interventions on behalf of Liverpool Council, undertaking Care Act Assessments, commissioning services where applicable, undertaking Safeguarding investigations, and providing Social Circumstances reports to the Mental Health Review Tribunal for patients detained in hospital.

Requirements for this Community Care Co-ordinator role:

* Minimum 2 years post qualification experience
* HCPC Registered
* Enhanced DBS on the update service
* Social Work Qualification
