My client is looking for a dedicated, organised and hardworking Care Coordinator to join their busy domiciliary branch based in Nottingham. Your role will be influential in the successful care delivery of a much-needed service caring for vulnerable people in their homes.

The Care Coordinator's main responsibilities are to

  • Ensure packages are sourced and managed into the business.
  • Coordinate, plan and produce staff rotas in advance, ensuring adequate staff cover.
  • Maintain a rostering system to capture changes as soon as possible on the system.
  • Receive requests to provide domiciliary care service, and liaise with the referrer
  • Liaise with other healthcare professionals and ensure we are passing necessary information to other professionals in a timely manner.
  • Maintain the rostering, holiday schedule and visit completion for the payroll and accounting systems.
  • Manage the on-call cover outside of normal working hours and maintain a detailed log of all on-call activity.

The successful candidate will have

  • NVQ Level 3 Health and Social Care
  • Experience working within domiciliary care as either a Supervisor or Coordinator
  • Strong organisational and time management skills
  • Ability to react swiftly to ever changing circumstances
  • Excellent work ethic to manage an out of hours on call system
  • Ability to work under pressure
  • Punctuality to complete essential tasks and duties within allocated timeframes
