I am currently looking for an experienced Approved Mental Health Practitioner who is immediately available to join an outstanding London based Social Work Team!

It is essential that the Social Worker has experience within a full range of adults and children's' social care responsibilities including, children and families, learning disabilities, physical disabilities and Drug and Alcohol services.

The desired Social Worker will have excellent knowledge of the NHS and Community Care Act, Mental Health Act 1983; and 2007 Amendments, Disability Discrimination Act, Carers Representation and Services Act, Human Rights Act, Children's Act.

Responsibility of the Social Worker:

  • To ensure key tasks and priorities set within national and local strategies are prioritised and implemented by staff supervised.
  • Undertake adult assessments in accordance with the Care Act (2014)
  • Undertake adult support planning in accordance with the Care Act (2014)
  • Practice Under the Mental Health Act (MHA) 1983 (2007).
  • Work in partnership with the service users, carers and the wider MDT to coordinate appropriate care through CPA and care planning.
  • To deputise for the Team /Sector Manager as required.
  • To adhere to both the Trust and Local Authority's policies and procedures and promote the principles of equal opportunities and anti-discriminatory practise.

You must have:

  • Social Work England Registration.
  • A clear, enhanced DBS background check.
  • At least three years of post-qualification experience, of which one year must be working as part of an MDT.
