Adult's Social Worker

We are looking for a Social Worker to join a Discharge to Assess Team.

THIS ROLE REQUIRES A SOCIAL WORK QUALIFICATION WITH A MINIMUM OF 2 YEARS POST QUALIFIED EXPERIENCE.

About the team

This team specialises in supporting vulnerable adults to sustain or improve their wellbeing and quality of life. A key aspect of this position is to work in a multidisciplinary fashion alongside health and social care and the voluntary sector to support service user independence when appropriate. Initial assessments within a 24 hour period is required to ensure service user safety and decide the pathway most suited to them.

About you

The successful candidate will be a professional well versed in MCA assessments and D2A pathways. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work setting are essential for this position.
